Welcome, and thank you for choosing Eagle Creek Dentistry for your dental care. We take a comprehensive, approach to dentistry, performing a through evaluation with each patient. As our patient, you are more than just a mouth. We take the time to get to know you, along with your medical and dental history. We listen to your dental needs and aesthetic goals, so together; we can create a treatment plan to give you a beautiful, healthy smile that lasts.

Your initial appointment will be a complete Comprehensive Exam, which usually includes taking any necessary x-rays, an oral cancer screening, Dental caries (cavities) screening, Occlusal analysis to examine the position of the bite, Muscle exam, Joint exam, and Periodontal exam.

Dr. Valentine will spend time with you to discuss your dental concerns and goals. When we perform an oral exam, we aren't just looking at your teeth, we examining the bite relationship, health of the gums and structure of the teeth while looking for hidden signs of stress and instability.

There is no one-size-fits-all solution in dentistry. Dr. Valentine will present you with all of your options and help you choose the treatment that will deliver you with the best results that are long lasting, comfortable, natural-looking, and best for your budget.

Please assist us by providing the following information by the time of your first appointment:

  • Completed patient forms and health history (forms will be sent electronically).
  • A copy of any current x-rays (can be e-mailed to us in JPG format at [email protected])
  • A list of medications you are currently taking.
  • If you have dental insurance, please provide our administrative team with the necessary information prior to your appointment. This will allow us to verify your coverage and benefits in advance, saving you valuable time and allowing us to help you process any claims.

IMPORTANT:

All patients under the age of 18 years of age must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to dental treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.)

X-Rays

X-Rays are necessary to do a proper exam, as we cannot see the inside of your teeth without them. If you have current x-rays from your previous dentist, please request that they forward them to our office. This will save you money, as we do not want to take any x-rays you don't need. For your convenience they can be e-mailed in JPG format to [email protected]. If additional films are necessary, they can be taken at our office.

What is considered current x-rays?

Full set (FMX) and Panoramic are current for three years.

Bite Wings (BW), and individual films (PA's) are current for 12 months.
